Life Insurance - Living Benfits Options
Plan pays 60% of the Life Insurance benefit to any life insurance policyholder who has been diagnosed to be
terminally ill by a California State Licensed Physician “Terminally ill” means 12-months life expectancy.
How Do I Get Started
In order to process a claim on your behalf please mail us the following:
- Please complete the Request for Payment of a Group Life Accelerated Benefit form
- Ask your doctor to complete the Attending Physician's Statement - Accelerated Group Life Benefits form
- Request Medical Records

The policy pays 60% of the total benefit amount.
The 40% left will be paid to the appointed beneficiary upon the death of the insured.

The United States Life Insurance Company usually pays the Living Benefit Option in one or two months after the receipt of
the Claim Form.
This is the case for the 98% of the claims.
Delays Can Happen:
- Delay in getting the Medical records
- Insufficient evidence of 12-month life expectancy
- Contestability: If the claimant has been insured for less than 2 years, the Insurance Company will investigate the accuracy of the application’s statements. This procedure will take two or three months, depending in how diligently the Doctor provides the Medical Records to the Insurance Company.
